Nomination Forms - Mayor and Councillor

All candidates are required to complete and submit the following three forms to Elections Beaumont:

1. Nomination Paper and Candidate's Acceptance: Form 4

2. Candidate Information: Form 5

3. Candidate Information Disclosure Form: CIDF

Trouble opening or completing PDF forms?

Fillable forms do not open properly on some mobile devices and web browsers. To fill in and save a form:

  1. Click on the PDF link to save it on your computer.
  2. Launch Adobe Reader.
  3. Open the PDF from within Adobe Reader. You can now fill and save your form.

Nomination Forms - School Trustee

Nomination forms for trustee for Black Gold School Division can be found on Black Gold's website: Becoming A Trustee.

Nomination forms for trustee for The St. Thomas Aquinas Roman Catholic Separate School Division can be obtained at their Division Office at 4906 50th Avenue, Leduc.

Nomination Checklist

Complete required forms.

  • Ensure you have completed Form 4, Form 5 and the Candidate Information Disclosure Form.
  • Form 4 must be signed by:
    • A Commissioner of Oaths or Notary Public for Alberta, or
    • The Returning Officer or delegate (available in person, by appointment only)
  • Form 5 should include campaign banking information. Do not use personal banking information.

Review campaign finance obligations

  • Review new requirements for 2021.
  • Obtain your own legal advice.

Obtain the required number of signatures.

  • Persons signing a candidate’s nomination paper must be an eligible elector (i.e. 18 years of age and a Canadian citizen, residing in Alberta; the person’s place of residence must be located in the local jurisdiction on Election Day).
  • Original hand-written signatures are required. Digital signatures are not accepted.
  • For mayor and councillor candidates, a minimum of 5 signatures is required.
  • For school board trustee candidates, a minimum of 5 signatures is required.
  • It is advisable to have more than the minimum required number of electors sign the form in case one or more signatures are not eligible to nominate.

File your nomination forms with Elections Beaumont.

For Mayor and Councillor positions, all candidates must provide Form 4, Form 5 and the Candidate Information Disclosure Form, completed in full, to Elections Beaumont before noon on September 20, 2021.

Due to COVID-19 physical distancing precautions, Nominations Forms will be accepted by appointment. Call 780-929-1352 or email city.clerk@beaumont.ab.ca. Please ensure that forms are filled out ahead of time; they can be reviewed by a staff member at your appointment.

For Public and Separate School Trustees, forms should be submitted to the individual school boards.
